SHBG, or sex hormone-binding globulin, binds tightly to testosterone, preventing it from entering cells and activating androgen receptors. This means that even if total testosterone levels are adequate, high SHBG can lead to low free testosterone levels, which may result in symptoms of hypogonadism.
